WAIVER & RELEASE OF LIABILITY/ALL ATHLETES READ CAREFULLY
WARNING: THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO SUE PAW PAW LAKE YACHT CLUB AND OTHER SPONSORING PARTIES OF THIS EVENT. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY.
In consideration of Paw Paw Lake Yacht Club (“PPLYC”) allowing me to participate in this July 12, 2025 Triathlon, I ______________________________ for myself, and on behalf of my spouse, children, parents, guardians, heirs, and next to kin, and legal and personal, representatives, administrators, successors, and assigns, hereby waive and release any and all rights and claims for damages I may have against PPLYC, or any other sponsoring party of the Triathlon. I further attest for any and all rights and claims for damages suffered by myself arising out of my participation in the Triathlon and to the best of my knowledge, I am in good health and in proper physical condition to safely participate in this Triathlon and that no qualified individual has advised me against competing in any/all portions of this Triathlon. I further agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate in this event.
